PAYMENT TERMS |

Payment for your initial order must
be made by credit card (VISA, Master Card or American
Express), unless some other form of payment is arranged
between you and FORLIFE in writing. You may set Net 30
Days term after your credit references are submitted and
approved by FORLIFE.
Minimum purchase for opening account is $250.00, for reorder
$50.00. We will charge 1.5% per month on over due balance
and $20.00 for returned check. |

RETURN POLICY |

If you found breakage or defect with
your purchase or all other claims, please report us within
10 days in writing. Please send claim truck shipment damage
to the trucking company.

SHIPPING & HANDLING |
| |
The majority of our orders in United
States are shipped via UPS Ground. Most in-stock items
will be shipped out to you 3-9 business days after receipt
of your order.
Shipments to Hawaii and Alaska will be sent via Priority
Mail of United States Postal Service.
We will add $1.00 handling charge per each carton.
If you prefer another shipment method, please provide
your routing guide in writing. |
